I don't think it is demuxer's problem(qtaac enabled) because I have changed various kinds of video sources for testing.
The .mov file is a 1920×1080 DV clip

No I didn't mean that one particular demuxer should be blamed ( though it is still possible if libavcodec fails, which would result in failures in nearly all popular free source filters ), I was thinking if the source filter itself is broken, or not compliant to satisfy most decoders. For an unrealistic example, it uses 13-bit depth which keeps within spec but can still challenge most decoders, if not all. I have to doubt about it because not only your system changes before/after installing the hotfix, but your souces varies unless you did exactly two encoding on the very same souce with same audio settings, which seems unreasonable to me so that I cannot help to avoid making such a silly assumption.

May be this is the final solution: remove that KB2670838 hotfix
with this

and everything works well :)
I'm sure that during these days there are no more system changes except windows update(I have checked the system log in the Solution Center)
Maybe it is an incidental only happens to me.

貌似这个补丁会造成部分DELL和HP的笔记本电脑出现崩溃，原因貌似是与板载AMD显卡存在冲突。
目前唯一的解决方法就是将其卸载、隐藏，然后等MS发布新版补丁。

Does x264 --demuxer lavf -o output.mp4 input.mov works again after removing that update?

No matter how system changs, lavf showing read error for a normal mov file is never intended and should be considered as a regression. Unlike qtaac depending on msvcrt, lavf demuxer does not require special system resources and generally won't be affected by such kind of updates.

After reading your reply I tried again deal with that DV clip with qtaac & lavf
You are right, lavf failed with that .mov file, but seems works well with other video source.
That's odd.
